New Passport
Requirements:
A DS-11 form is accomplished for those applying for a new passport which applies to:
- damage to your previous passport.
- This is your first application for a U.S. Passport.
- An application for an applicant under the age of 16.
- If last passport issuance was 15 or more years ago.
- If your last passport was issued when you were 15 years of age or younger.
Passport Renewal
Requirements:
A DS-82 form is accomplished for those applying for a passport renewal. Please note that you will need to be in possesion of your previous passport, it needs to also be undamaged, and issued when you were 16 years of age or older.
- Your previous passport must be issued when you were 16 years of age or older.
- must not be damaged and you will need to be in possession of your previous passport
- passport’s blank pages is limited
- passport is close to expiry, or has expired within 5 years.
- passport was issued a year or more ago and a legal name change has been made.
Lost or Stolen Passport
Requirements:
A DS-64 is accomplished for those whose passports are applicable to one of the following:
- If you would like to report your passport as lost or stolen
- If you would like to report your passport as lost or stolen AND submit an application for a new passport.
